Syracuse University's Institute for Veterans and Military Families (IVMF) would like your participation in a short survey about coordinated care systems for service members, veterans, and military-connected families in New York State.
The Institute for Veterans and Military Families at Syracuse University is the first interdisciplinary national institute in higher education focused on the social, economic, education, and policy issues impacting veterans and their families. The IVMF supports communities through collective impact efforts to enhance delivery and access to services and care to the military-connected community. Over the last ten years, more than 150,000 transitioning service members, veterans, and their families have leveraged the IVMF's portfolio of career development, credentialing, business ownership, and community-based services and programs as a bridge from military to civilian life.
They are currently exploring an opportunity for a statewide network of providers that would secure a single point of entry for veterans and their families to get connected to health and human services they need in the communities they call home. They previously interviewed about half of the County Veteran Service Officers in New York. They appreciate all of the feedback they've received so far and used those interviews to help guide their focus group discussions, which helped them better understand how organizations in each region collaborate to serve veterans and their families. They believe your input will be critical to informing any future plans to connect regions across New York State and hope you take the time to complete this short, 10 minute survey.
