NYC DVS Aftercare Specialist Position

The NYC Department of Veterans' Services is seeking an Aftercare Specialist.  This is a full-time position with an annual salary range of $54,100-$83,981. 

The Aftercare Specialist will perform ongoing follow-up and case management with formerly housing-insecure veterans to ensure that they avoid housing insecurity/homelessness recidivism.

Reporting to the Director of Aftercare Services, the Aftercare Specialist will:

Provide direct assistance to ensure housing stability for formerly homeless and at-risk Veterans and Military Families through critical time intervention outreach phone calls and emails.
Follow established aftercare protocol, contacting Veterans to ensure a smooth transition into housing and identify current or potential needs for them to maintain permanent and stable housing.
Ensure timely and accurate input of all case notes into the VetConnect NYC platform.
Provide guidance and referrals to Veterans with an emphasis on developing healthy lifestyle habits, goal fulfillment, and self-sufficiency.
Make appropriate referrals to Care Coordination Unit (CCU) and Housing Support Services (HSS) as needed.
Manage the Veteran aftercare database, and other tools that support DVS' aftercare and prevention work as needed.
Stay current on all available resources available to veterans and military families
Provide raw data to supervisor as needed.
Assist in the planning, implement and support of special initiatives and projects that support current or former clients as it pertains to aftercare and prevention.
Attend DVS special events, community meetings, and inter- and intra-agency meetings as needed to support DVS' mission and vision.
Represent the Aftercare and Prevention Department as needed in meeting, conferences and/or events.

Minimum Qual Requirements:

A baccalaureate degree from an accredited college and two years of experience in community work or community centered activities in an area related to the duties described above; or
High school graduation or equivalent and six years of experience in community work or community centered activities in an area related to the duties as described above; or
Education and/or experience which is equivalent to "1" or "2" above. However, all candidates must have at least one year of experience as described in "1" above.
Preferred Skills:

Multiple years of experience in social services case management.
Proficiency in Microsoft Office software and knowledge and familiarity with databases.
Must have a proactive, collaborative approach, be a team player and have a positive attitude.
Excellent oral and written communication, organization, time management and multi-tasking skills.
Knowledge of, and experience with, New York City housing and public assistance systems preferred.
Experience and/or familiarity with veteran and military family resources, specifically related to housing.
Veteran status a plus.
