The Department of Pediatrics at the Children's Hospital at Montefiore/Albert Einstein College of Medicine seeks a motivated, highly organized individual to serve as a Community Health Care Worker to provide asthma education and support activities of the NIH school-based research study. This is a full-time position.
Responsibilities:
Schedule and conduct asthma lessons for parents and children with asthma, either remotely or in-person as per study protocol and procedures
Revise and develop asthma lesson materials and other study-related materials, as needed
Provide asthma care coordination and referral to community resources
Gather available community resources
Collect and enter data, maintain appropriate documentation, and prepare reports
Assist with recruitment of study subjects and survey completion
Communicate with schools and health care providers
Assist with other study-related tasks or other asthma studies within the team, as needed
Other duties as assigned
Qualifications:
Bilingual (English/Spanish) required
Experience working with children and families, community engagement, care coordination, case management is required
Member of the Bronx, NY community is highly preferred
Knowlegde of asthma and research is highly preferred
2 year college/Associates Degree in health education or equivalent is required
Community Health Worker certification preferred
Bachelor's degree preferred
Experience may be substituted for education
